Welcome Letter Form:
Welcome to Internal Med ID Associates. We hope that your health care experience with our practice is excellent. In order to provide you the best care we request some assistance from you.
We ask that all patients present with a current insurance card, a valid photo ID, and copays are due at the time of visit. If you are unable to make your appointment, please allow for a 24 hour notice, otherwise you maybe subject to a fee. All new patients MUST have records on site. If records are not received from your referring doctor, the appointment may need to be rescheduled. Please contact us to ensure records have arrived on your behalf.
The group accepts most insurances. Please contact your insurance provider to review your policy for office coverage. If you have a high deductible policy, a portion of the claim will be collected at the visit. We accept cash, check, Mastercard, Visa, and flex cards. If you have Medicaid secondary to an HMO and your HMO requires a copay, you will be billed if Medicaid does not cover the copay.
Our office utilizes an Electronic Medical Records system. We have the ability to E-prescribe medications, electronically send and receive Quest Diagnostic labs, upload radiology results from most facilities in the area and each patient will receive a clinical visit summary at the completion of the visit.

A general information sheet will be mailed to the home address. Please complete this form and bring it with you to your visit. We encouarge patients to bring a list of their current medications and a list of doctors involved in their care. A pharmacy phone number would assist us in submitting to your correct pharmacy.
